Support coordinators, plan managers, allied health professionals, LACs, GPs, schools and families all send young adults our way. However you have come to know one, you are looking for somewhere they can land and actually stay. We would rather give you a straight answer about fit than win a referral we cannot serve well. Here is who we are built for, and how to tell if someone is right for us.

What we are built for

The brief, in four lines

A specific cohort

Young adults 18 to 40 with intellectual disability or autism who are socially capable but under-connected. The ones who can hold their own in a group, once they have a group.

One connected pathway

Social programs, respite and supported living sit on a single pathway with the same team throughout, so a young person is not handed between strangers as their needs change.

Nurse-led delivery

Rachel, our founder, is a nurse. Health and wellbeing changes get noticed early and flagged to you, not left to surface at review time.

Genuinely local

Based in the Redcliffe to Caboolture corridor with door to door pickups, so attendance holds up and your plan goals actually get worked on week to week.

Likely a good fit

A young adult 18 to 40 with intellectual disability or autism who is missing connection more than they are missing care. Someone who does better with the same faces week to week, who would enjoy a crew their own age, and whose family is quietly hoping the world opens up a bit. Often the social side is the way in, with respite or a home of their own somewhere on the horizon.

Probably not us, and that is okay

Someone whose primary need is complex clinical or high physical care, or who is looking for a solo 1:1 arrangement with no interest in a group. We would rather say so early and help you find the right door than take a referral we cannot do justice. Point us at a fit and we will make room.

Supported living

We have room for a third housemate

Our supported independent living home currently has space for one more young adult, and finding the right match matters as much to us as it does to the two already there. We are looking for someone 18 to 40 who would get on with a small, settled household and who is ready, or getting ready, for a place of their own.

It does not have to be an immediate move. Some of the best fits start on a weekend program, try a respite weekend, and grow into the home over time. If you have someone in mind, tell us about them and we will talk you through the household and the match.

How referring works

    Send us a note

    A short message with the young person's situation and goals is plenty to start. No forms to chase first.

  2. STEP 2

    We give you an honest read

    We tell you plainly whether we are a good fit, and if we are not, we will point you toward who might be.

  3. STEP 3

    A gentle first visit

    Usually a program visit or a low-key meet, so the young person and family can see us before committing to anything.

  4. STEP 4

    Settle in, then build

    We start with the social side and let the pathway open up from there, keeping you in the loop as goals progress.
